Our Pharmacy module allows the timely delivery of medicines on the day of discharge and the tracking of all admissions to support effective and auditable medicine reconciliations within 24 hours of admissions.

It can also be used to request formulation changes to medicines and counselling from the Pharmacy teams.

By being able to see the demand across the whole hospital, use of scarce pharmacy resource is greatly improved.

Electronic Referrals
Managing referrals using Cayder PFM removes the need for phone calls, faxes or emails. Referrals can be made at any time of day or night and the referral is clearly tracked from the instant that it is requested through to completion. Checking the progress of a referral can be done whenever it is convenient. The PFM ensures that referral information is consistent and complete.

Can Cayder PFM help with the NICE/NPSA targets of medicine reconciliation?

NICE/NPSA guidelines recommend that pharmacists are involved in medicines reconciliation as soon as possible after admission, ideally within 24 hours. Our pharmacy are struggling to locate all new admissions within this time - can you suggest a way of addressing this?

A simple solution to your problem would be to use a pharmacy virtual ward in Cayder PFM automatically populated with all new admissions. Use a simple tick box to see whether a medicine reconciliation has taken place for each patient, then on a hospital wide view display all patients for whom this hasn't happened. The pharmacy would then be able to see in real-time which patients need a medicine reconciliation and where they are. The PFM could also be used to report on how well the trust is managing to complete medicine reconciliation within the 24 hours target. To make the process really efficient, pharmacists could up-date the medicine reconciliation status at the bedside using a mobile device.
